We request you to specify how you would like your donation to be used. In order to clarify this, please print and fill in one of the following forms, and mail it with your check. If you would like your donation to be used to support:

  • JKP building and relief projects in India, please fill out and return this form: JKP Bldg or Relief.
  • Physical repairs, maintenance or improvements to Shree Raseshwari Radha Rani Temple or other buildings or grounds at JKP Radha Madhav Dham, please fill out and return this form: JKPRMD Bldgs etc.
  • Religious activities of JKP Radha Madhav Dham, including devotional teachings and other religious programs, celebrations and outreach, please fill out the form: JKPRMD Religious Activities.
